What full RCM actually includes
Full-service RCM starts before the visit with eligibility verification and prior authorizations, continues through coding and claim submission, and ends with denial management, A/R follow-up, payment posting, and patient collections. Reporting ties it together: you should see denial rates, days in A/R, and collection rates regularly.
Partial outsourcing is common too. Many practices keep coding in-house and outsource the back end, or outsource everything except patient collections. Define the scope explicitly in the contract, because gray areas become blame areas.
When outsourcing makes sense
Outsourcing tends to win when billing staff turnover is chronic, when growth has outrun your back office, when collections have plateaued despite a full schedule, or when the practice owner is spending evenings on billing problems. Those are all signs the function has outgrown DIY management.
It tends to lose when the practice is small enough that one good biller handles everything, or when the real problem is documentation and no vendor can fix that from the outside. Be honest about which situation you are in.
Pricing models, plainly explained
Most RCM vendors charge a percentage of collections, commonly advertised in the 4 to 10 percent range, with higher percentages for smaller practices or complex specialties. Some offer flat monthly fees or per-claim pricing, which can favor high-volume practices.
Whatever the model, clarify what counts as collections, what the minimum monthly fee is, which services are included versus add-ons, and what the termination terms look like. The contract matters more than the headline rate.
Red flags in RCM vendors
Watch for these warning signs when evaluating an RCM company.
- No transparency into your own A/R. You should see every claim, every denial, every action taken.
- Long-term lock-in without performance clauses. If they are confident, they will accept termination for missed targets.
- No dedicated account team. Shared queues mean your practice is nobody's priority.
- Vague reporting. Denial rates, days in A/R, and net collection rate should arrive on a schedule, unprompted.
- They cannot name clients in your specialty. RCM is not fully transferable across specialties.

If collections are flat, A/R is aging, and reporting is vague, you have a vendor problem regardless of what the monthly calls sound like.
What should an RCM contract include?
Clear scope of services, the exact definition of collections for percentage pricing, minimum fees, reporting cadence and metrics, performance targets with remedies, data ownership and return on termination, and termination terms including notice period. Never sign an RCM contract without termination-for-performance language.
