Think of billing as a subset of RCM. A billing-only vendor typically handles coding review, claim submission, payment posting, and basic follow-up. A full RCM vendor adds the front end (eligibility verification, benefit checks, prior authorization management) and the heavy back end (denial management with root-cause analysis, A/R follow-up, payer contract support, and performance reporting).
The distinction matters because most revenue leaks happen outside the billing step: eligibility errors, missing authorizations, and unworked denials. If your problem is front-end leakage, a billing-only vendor will not fix it no matter how good their claim submission is.

How to spot a billing-only vendor using the RCM label
Ask what happens before the claim is created and after it is denied. A true RCM vendor can describe their eligibility workflow, their authorization process, their denial reason tracking, and their A/R follow-up cadence in detail. A billing-only vendor wearing the RCM label will be vague on the front and back ends and fluent only in the middle.
- Ask who verifies eligibility and when, and what happens when coverage is inactive.
- Ask who manages prior authorizations and whether it is included in the quoted rate.
- Ask how denials are tracked, appealed, and reported, and what the escalation rules are.
- Ask for monthly reporting on the full cycle: clean claim rate, denial rate by reason, days in A/R, and net collection rate.
